To start this quest you need to have completed two of the other quests located near The Gracious Hand in the Slums of Vernworth. Once I had completed The Gift of Giving and The Heel of History, I was able to start Saint of the Slums. You’ll know the quest has activated once you see Abbess Elena arguing with a couple outside The Gracious Hand.

Collect three Miasmite

The first step of Saint of the Slums in Dragon’s Dogma 2 will trigger when talking to Elena. She’ll ask you to go and collect three Miasmite. Luckily this material isn’t so hard to collect. All you need to do is venture out into the woods at night and attack the wispy ghosts you see along the way. They look like big pale clouds and they’re pretty hard to miss. Make sure to collect the resource they drop as this is Miasmite.

Take a Tour of The Gracious Hand

Take the Miasmite back to Elena and she’ll take you on a tour of The Gracious Hand. It’s only a small building so it doesn’t take long. Then at the end of the tour, you’ll be introduced to Lubomir who will kickstart the real point of this quest. He’ll ask you to investigate Elena as something suspicious is going on.

After this conversation, leave the building, and a conversation with Lottie will begin. After that return to Lubomir and he’ll tell you to investigate the basement. He specifically wants you to go to the basement sickroom which was only open at night for me.

Pass time until the night and head to The Gracious Hand, going left at the entrance to access the basement. Then take the door on the right and continue through to reach the sick room. Head behind the curtain and take both the Unlabeled Medicine on the shelf as well as the Records of Treatment.

Take the evidence back to Lubomir and he’ll then ask you to speak to two former patients of Elena and a doctor residing in the Checkpoint Rest Town to the west. I found both patients at Walter’s Tavern in the Slums at night.
